Considered 14+ years

$375 per year

  • 2 Routine hygiene cleanings

  • 1 Routine series of radiographs

  • 2 Regular exams with Dr Phelps

  • 1 Panoramic radiograph as prescribed by Dr Phelps

  • 1 Emergency/Limited exam

  • *each additional routine or maintenance cleaning is $140

Terms and Conditions

If paying with credit or financing card, less 5% discount will be given to cover administrative and transaction processing fees.

Loyalty Plan 15% discount does not apply to IV Sedation or general anesthesia.

If any benefit has been used, no refunds are available.Payment for treatment is due at time of service.  A deposit will be required for certain procedures.  Our 48-hour cancellation policy is enforced. If a patient does not comply, the scheduled cleaning will be forfeited.  Our plan is not insurance or a qualified plan under the Affordable Care Act. This plan cannot be combined with or used if you have dental insurance.
