Are you accepting new patients?

Yes. We're happy to accept new patients. We're always excited to provide new and existing patients with cutting-edge dental care. We look forward to meeting you (or your family members and friends)!

How do I schedule an appointment?

You're welcome to call our office at (480) 832-3400 to schedule your appointment or request one online, and we'll contact you to confirm your selection.

What is your cancellation or rescheduling policy?

We request that you contact our office at least 24 hours prior to your appointment.  Anything less than 24 hours prior to your scheduled appointment will be considered late cancellation. Late cancellations and no-shows are subject to the following fees to help offset our business operating expenses:

  • On the Doctor’s schedule: $100 per hour scheduled
  • On Hygienist’s schedule: $50 per hour scheduled

What do I need to bring to my first appointment?

You'll need to bring in the following:

  • Your completed new patient forms
    We can help you with these in the office if needed.  Please come 15 minutes prior to your scheduled appointment time if this applies.
  • A photo ID
  • Your insurance card
  • A list of current medications and medical conditions
  • The phone number of your primary care physician
  • The name and phone number of your preferred pharmacy
